If the three Task Hazard Rating tables are not open, it is because they haven’t been created. These tables can be created by setting up the Task Hazard Rating Worksheet report.
You might need to set up the cross reference tables for the Task Hazard Rating worksheet. If you do, follow these steps:
1.Click on Cross Reference on the Main Menu and select Open.
2.On the Cross Reference Tables window, click the New button.
3.Under Text enter "Task Hazard Rating Score: Probability" and under the User-Defined ID enter "HAZPROB", click the Shared radio button, and click OK.
4.Follow step 3 for the following two cross reference tables:
a.Task Hazard Rating Score: Repetition; HAZREP
b.Task Hazard Rating Score: Severity; HAZSEV
Usually, the Task Hazard Rating tables have already been set up. If you have to create them using the steps starting in the “Task Hazard Rating tables” section (above), you will need to add items in each table.
If you need to add items to any of these three Cross Reference Tables to support your own rating scheme, you must enter a number followed by a space and then the text, when entering the item’s text.
For example, the item “1 Less than daily” under the “Task Hazard Rating Score: Repetitiveness” table begins with the number 1 followed by a space. Use this format if you need to add items.
VISION will use the number to calculate the Hazard Rating when the “Hazard Rating: Task Hazard Rating Worksheet” report is generated. These three Hazard Rating tables are the only ones used by VISION whose items must begin with a number.
There are three tables in VISION that users will use if they intend to rate their tasks for hazard (Task Hazard Rating Worksheet and/or the Hazardous Task Analysis Worksheet). To create these tables, follow these steps:
1.On the Main Menu click on Cross Reference and then Open.
2.In the Cross Reference Table for Project window, click on the New... button.
3.In the New Cross Reference Table window, you will need to enter the title and User-Defined ID for each table, as follows:

4.Double click the first table, VISION will ask to arrange it, click "Yes", then click the New Item button.

In each of the table items above, the rating number that precedes the text (description) is the number that the system uses to calculate the hazard rating. Always follow this number by at least one space. If you have your own rating system, you may substitute your own numbers and descriptions. The users use these tables by linking one item per table to a task. VISION calculates the hazard rating by adding the numbers (that precede the description) of each item: Hazard Rating = Severity + Repetitiveness + Probability. Critical tasks are those having a hazard rating greater than 7 (using the above values).
The hazard tables are the only three tables used by VISION whose items must begin with a number.
It’s a good idea to close each of the three Task Hazard Rating tables after completing the Task Hazard Rating process. VISION does not automatically close tables between activities. If you do not close them, the tables may clutter up the display during the next activity. For more information on using Cross Reference Tables, see Setting up Projects and Work Areas.
