The Action Tracking feedback feature is an add-on module that gives learners a way to submit requests or report problems with the content that they are using. Your instance of VLS may not have it enabled.
Remember to be as specific about the action item possible so that it's easier for administrators to locate and fix the problem.
1.Click the Add Feedback icon 
in the top menu.
2.Enter a Subject for the action tracking item. Be as specific as possible.
3.If you belong to more than one Organization, choose the one best associated with this action tracking item.
4.Enter a description of the problem or suggestion. Be as specific as possible so that administrators and developers can find and fix it.
5.Click the Add Action Tracking Item button.
